you should be able to get them at any post office or service canada office

You can get them at Service Canada, Canada Post and at Passport Canada offices.  You can also do it a few ways online.

You can print the forms off the Passport Canada site.  

Or, my recommendation....use the "E-pass" option if you are going to take your application to a Passport Canada office.  The printout will have a bar code and the line ups are less for that kind of form.
