Do most airlines make them pay for a second seat?

Say you're flying with major carriers such as US Airways, Delta, Continental, United, American. Who is best at accommodating larger passengers on flights from east coast to the west coast?

I don't think there is anything they can really do once the overweight passenger has already checked in, gone through security, and is sitting in the seat.  I was sitting next a rather larger man on a flight from Georgia to Arizona, and all i could do was use the other arm rest and lean the other way. Very uncomfortable flight, but I didn't have the guts to request a seat change :T

In this case you will definitely need to pay for an extra seat. However some airlines do not charge exactly double air fare but they can favor you to charge just 50% extra (or different percentage depending on airlines policy) for an extra seat.

Airlines also have stretcher passengers accomodation. But that are only provided if passenger is MEDA (Medical Patient)

So get ready to pay for an extra seat :)
