When I say that I understand?

Shall I say "I understand" or "I'm understood"

What's the difference? I don't understand

"I understand" means that you understand what others are saying.

"I'm understood" is that other people understand what you're saying.

"I understand" means when you understand something that someone else said.

"I'm understood" means that the person you are talking to understands what you said.

Simply put -

I understand = I get/got the meaning; I comprehend; the message is loud and clear.

I'm understood = people get my meaning; they comprehend me and/or my actions; they get the message loud and clear from me.

Edit - example of sentence -

I understand : When your teacher asks you: Do you understand the lesson? You reply: Yes, I understand.

I'm understood : After you have delivered a message, your friends will nod their head in agreement, right? That's when their action of nodding their head is like a message to you to say that they understood what you are trying to say. So you can tell your teacher that - I'm understood (by my friends).
