    Yes, you must answer questions honestly.  Chronic depression is a mental illness.  I think you can simply answer depression.


  2. Yes-tell him. They want to know all types.  They will determine if it will affect anything--if you don't tell them and they find out  you could be held in contempt of court. Not good thing.

  3. They are referring to a mental disorder that prevents you to interact properly with your environment or that is painful .

    If your chronic depression prevents you from having a "normal" social and professional life then yes, you should report it.
