When a patient is transferred from general hospital to a rehab hospital?

are instructions concerning that patient sent with them? The nurse(s) asked me questions that should be written in the discharge papers. This left me very apprehensive about leaving her when visiting hours was over. We do peritoneal dialysis (PD) at home on the cycler and in the general hospital hardly any nurses knew how to do manuel PD. I think they may have been to rough handling her access tubing also. She can't speak because of head trauma so I need to be there to speak for her but can't all the time.

There should be.  When I went from a Medical Center to Rehab, I had a huge chart.  The patients attending physician will work with the Rehab hospital.
