When applying for a Passport did you encounter this problem? (Dual Citizenhip)?

Okay here is the question. Applicant wants to renew his US Passport. The problem is, applicant reacquired Philippine Citizenship, making applicant a dual citizen. According to US Passport Renewal Application, if applicant has performed certain acts, such as acquiring another nationality, they have to attach supplementary explanatory statement under oath attesting to his other citizenship. My question is...

If you have been in a similar situation, did you have trouble renewing your US passport?

Did State Department give you a hard time?

What information exactly is needed in the supplemental explanatory statement?

In response to Guest2433, is there going to be a problem in the future if you ignore this section about acts (e.g. reacquiring former citizenship) and they find out that you did after the fact.  What are the consequences vs declaring the fact like what Guest8616 suggested.  I am very interested in this matter since I for one is in the same situation and is about to renew my US Passport on 2013.
