like usually once a month... or every 2 months for a 12 month policy premium

Typically a semi-annual policy is due semi-annually and annual due annually.  If you don't pay it all at once, they tack something extra on the monthly payments.

I remember when I had a semi-annual policy and my insurance company sent me a notice "Congratulations, you qualify to pay your insurance annually" (like it was a good thing).
