When does goku first go Super Saiyan 2 and in GT why didn't gohan ever go Mystic?

The first time Goku went SS2 was during his fight with Vegeta, during the World Marial Arts Tournament. And Gohan never went Mystic in GT for two reasons: 1. Because he was not the focus of that series, and 2. He really fell out of training after the Majin Buu fight.

We don't see the exact moment that Goku first goes SSJ2 because he supposedly did that by training in the Other World after Cell exploded. So, when we see him next seven years later he already has achieved SSJ2 (and 3 too ^_~), therefore pissing off Vegeta.

As for the Mystic "form", in DBZ we were told that it was going to be permanent (notice how Gohan's eyes change after his power-up and STAY that way) and that Gohan wouldn't need to go SSJ again because all that flashy stuff and transformations only resulted in the loss of ki energy. And GT is full of plot holes and c**p, so I assume that SSJ Gohan is a result of that.
