  1. I have seen it done so, but the phone book puts Mac before Mc.  I would go that route simply because it makes sense.  A before C, after all.


  2. Ah, now it's time to complicate your life just a little--there are different filing systems, with different rules.  

    Sometimes Mc will come after Mac.  

    Sometimes a space will count (that is, come before A), and sometimes, it will just be ignored.

    Sometimes a name like O'Hara will be filed with Ohara--sometimes, not.

    The important thing is to be consistent with whatever rules are established for that particular file.

    So, the answer to your question is:  "It depends."

  4. NO.  The exception would be the quirky rules of any particular company or person who chooses to change the rules of alphabetizing to suit their invidual needs or wishes. :-)
