  2. wardrobe seems to mess up quite often, it's funny to see the inconsistencies.

  3. Have a look in the credits for whoever is credited with "continuity".  I'm not aware of a specific job title though.

  4. Monitoring continuity is the job of the script supervisor. My uncle works as a script supervisor for a well known director out in Hollywood, which is how I know about the job.

A script supervisor's main duty is to document every detail surrounding movie scenes as they are filmed.
