  1. nothing or the name of the writer of the book

  2. There are in-text and end-text references.  In-text is when you quote the sentence exactly from the book e.g. Matthew (Surname of the author, year, page number) stated that " Water is life"  End-text is full reference in regard book e.g. Morris, M. (2008). Water in new world. Pheladelphia.: U.S.A.  (This is APA style)

    Good luck.

  3. It depends on the format you want to follow. But to follow MLA (usually the most common style especially for english) you would put down the author's last name and then the page number(s) in parenthesis. (last name, page number) But then you'd need a work cited page (aka bibliography) that will correctly cite the book you quoted from.

this link tells you how to make a work cited page. However, if you only have one book and your teacher does not care about a work cited page then you can just not do that.

  4. You could write the number 1 (if you are using more than one quotation) and at the foot of the page you could write 1 and then the name of the book you are quoting from and the author. If you are only using one quotation just put an asterisk after the quotation and another asterisk at the foot of the page with the references before mentioned.
