  1. An EKG is only required if you need a first class medical certificate. If you do, you need one first after you turn 35 years old (to establish a baseline), and then once a year every year after 40.


  2. One when you're 35, and one every year once you're 40.

  3. In addition to the answers above, they're also required when there is some question as to the condition of your heart, even for a third-class medical.

    If you've ever had any kind of incident or surgery related to your heart, chances are good you'll have to submit to an EKG on an annual basis to maintain your medical, again, even third-class.
