Question:

When is an exemption form needed for not vaccinating? Drs visits? or strictly for starting school?

I just want to know if I need a form for every dr's visit or are those forms JUST for getting kids into school? I'm confused.

  1. Just for childcare, preschool, and school.  Check your state's laws

    and see if your state offers a philosophical (personal beliefs) exemption or just religious.  If only religious is available, or you think you might move to a different state where only religious is available, tell your doctor now that you are religiously opposed.  If you move to a strict state, it would help a lot to have that in the medical chart that you are religiously opposed.
