  2. Yes, however the number of stars that a monster has indicates how many tributes it needs.

  3. yes, you cant just discard a card from your and summon a lvl 5+ monster.  however there are some cards like cyber dragon and a cards like chaos emperor dragon - envoy of the beggining which you have to remove 2 cards from play but the cards have to be in your graveyard.

  4. If you mean for a Tribute Summon for a card like Summoned Skull, yes.

    However, there are cards like Cyber Dragon and The Tricky that "cheat" this rule in a sense.
