I have roughly a year left of under grad studies to complete; I'd like to start graduate studies shortly thereafter.

  1. The fall of the year before you intend to start your graduate program, so senior year if you are still in school.

  2. Apply during the middle of the Fall semester of your senior year.

  3. Most grad school application deadlines are around the beginning of the year.

You need to look up the deadlines for the schools in which you are applying then decide when you will do the application.

I'm a senior in college that is currently applying to law schools.  Most of my deadlines are in Februaryish.  So, I'm going to start working on them now.  Fall semester of my senior year.....

  4. It depends on your major. Some majors require you to have work experience before you can apply for grad school. If you don't, then you do the same thing you would do when you were applying for undergrad, apply in the year before the semester you wish to attend.
