  1. The two are VERY different. If a man claims that he is Napoleon, he is having delusions. That is considered to be a delusion of grandeur, a symptom of schizophrenia. It could also be caused by another mental illness. If a man feels that he is really a man trapped in a woman's body, it is not a delusion because there is an anatomical reason for it. People that fell that they're born with the wrong gender have pituitary glands the size of the opposite s*x, therefore identifying with the opposite s*x. It's not a mental illness, because the reason is anatomical and not physiological. Medicines can not make a difference in this matter. There's a scientific reason for everything if you dig deep enough and not just judge people superficially, you know.
