Are you one of those lovley people who don't know what they are doing and taking the jobs of hard working americans?  How lovely to see that you know that CPT isn't the name of the form but the actual code you bill with.  You'll get them Christmas Day, because you just missed the Easter Bunny's delivery of them....  GOOD LUCK

??  You have to give us a little more info here of what you're asking?  Are you talking about HCFA/CMS forms?  and from who?

CPT in the medical world means procedure codes which indicate what was done in the office between the doc and you!  They put these codes on what is called a claim form and then it is submitted to the insurance company!
