  2. Even Wiki isn`t giving dates, but you would think that even in the times of Trafalgar or before, when there were a lot of boats in close proximity, some form of light system would be used to prevent collisions.

  6. In general, it's mentioned by Homer, as a means to keep fleets together.

    If you mean today's international standard system, it was proposed either in Washington DC in 1889, or St. Petersburg in 1908. I couldn't tell which from the reading, but it all came out from the first meetings of the International Society of Hydrographers. Today's standards were finalized in 1960 by the SOLAS (Safety of Life At Sea) convention.
