  1. Normally the entire form is required, so your signature will show in detail what you are consenting to.


  2. Yes you just need to sign the last page, but why not initial each page as well. i wil not say here why that would be sensible,, because I hope it is obvious

  3. Just recently I faxed back only the sheets that had my signature. That's all the other person is looking for. I was sent several pages, but all that was needed was my signature. The consent portion where you sign should state what you are signing for. Keep it for your records.

  4. When in doubt, send the whole thing.

  5. To be safe, it is better to fax back the entire pages involved with a cover sheet. You don't know if other faxes are coming through and yours might be lost in the shuffle. Otherwise, 'ASK" the person you are faxing what they would prefer - then you'd be safe.
