When you fill up insurance policies, do you have to state the status of the beneficiaries?

Like if your beneficiary is single or married.

And if the answer is yes, my follow up question is, If he got married without telling you, (so instead of single he is now married) what will be the problem?

(Sorry but I am not from U.S. and insurance is not a big issue where I come from... So I just want to understand about it, because I have a problem related to it.)
