Traveler has it right. If you arrive in HCMC or Ha Noi simply walk around and you will see lots of tourist / treking options. You might want to do some oneline research on the subject to get a feel for the quality of the different companies.

Many choices out of Chang mai in thailand.  Sapa has great treking in Northern Vietnam with home stays at various Hmong villages etc. Tours run from around $100 and go up to about $300 per person.  Check this site.
