  2. You can find all the necessary forms at http://travel.state.gov/passport/passpor...  You can fill out the form on line and then print it (forms can not be processed on line, you must do that in person).

    After you have your forms filled out you will need to take them along with 2 passport photos (available at Wallgreens, Rite Aid, Costco and other stores w/ a photo dept for $7-10) and a *certified* copy of your birth certificate to your local county court house or select post offices (not all post offices process application.  On the website listed above there is a list of places that process applications.

    There is a fee of $100 for your passport.  $25 is a processing fee paid to the agency you turn your paperwork into.  $75 is for the passport itself.

    Current waiting times are 4 weeks or less from the time you turn your application in.

  3. Go to a government agency such as a post office.  It'll be around $60 or so.
