Where c an I find an example of a patient information sheet?

I also have the same medical terminology class and need to know what a patient information sheet looks like, oso I can write one out and learn how to do one..

Also note I don't know if the sheets are done differently in UK or other countries from the USA or not are they?

Every country and even every institution in a country varies the OPD sheet considerably in form . The content and the primary aims remain the same, - to keep the records of the patient who is getting treated .

Basically when the comprehensive case Report CCR is made eight things are put on top :  Name, father / husband's name, s*x, Age, address , occupation, income and problem . Later every department that you visit and the treatment given is recorded . When you are an in-patient, Ther eis a whole lot of new sheets are attached which will remain in the department concerned .
