Where can I find a Prudential spousal waiver form?

My husband gave me a copy of the Prudential spousal waiver form to fill out and I messed it up. I need a replacement ASAP! I'm having trouble finding one online so I can print it. Does anyone know where I can find it? Thanks!

Xiomy, you may need to contact Pru for that. Also, before you sign it I believe it needs to be witnessed by either a company rep or a notary, double check me on that first. Good Luck,   ;-)
