I use InPhonex and CallCentric for my DID's (links through my profile).

If you can get a Linksys ATA, you can get DID's in USA/Canada for as low as $1.95/month + 1.98 cent/min outcalling and 1.5 cent/min incoming calls.  Calls to Jamaica are 8.1 cent/min with CallCentric.
