Where can I find a comment box for a webpage I'm making?

I'm making a web page for the place I work. My boss want me to add a page with a comment box. Where guests can leave a comment about the site or our store. I dont want a chat box type of thing. But I would like it if the comments would be displayed on the page where people are leaving them. All I've been able to find are ones where I have to go to a website to see peoples comments. But I would like all guests to be able to view them.

One where people could enter a name would be nice too. Not like logging in or anything. But just so it didnt have to be anonymous.

Its a business website for a clothing store. So nothing childish or funky please.

Any codes or websites where I can find one??
