Where can I find a connector that turn an 8 wire Cat 5e cable into 4, 2 wire RJ11 ?

I'm running 4 phone lines. from a punch panel and I don't want to use 4 different cables.

no big deal, from your patch panel, run your cat 5 to a RJ11 keystone jack and use the white blue on L1, the white orange on L2, the white green on L3 and the white brown on L4.
