  1. google

  2. You can't "find one".  You have to make one - that's the point of them wanting your plan.

You need to decide on a HS'ing method, and a curriculum - or curricula, that you'll be using for your child(ren).  Then make your yearly plan based on that.

The curricula you choose will have "scope & sequences" for you to use.  Just give the school district a copy of them.

  3. You might try using a book such as "Write Your Own Curriculum:  A Complete Guide to Planning, Organizing and Documenting Homeschool Curriculums" by Jennifer O'Leary.

Or try the website for the World Book Encyclopedia company

http://www.worldbook.com  They have a parent helps section that gives a "typical course of study" for every grade from pre-k to 12.  These are lists of the sorts of things that children typically learn at each particular grade level.  They might be useful in figuring it out.

Or you could use a purchased curriculum's "scope and sequence" if you're going to do it the more expensive way.  Most any publisher of homeschool materials (or of any textbooks) will be able to send you one, though they might charge for it.
