Where can I find an example of a "letter of Non-Impediment" so I can get married in Ukraine. I need an example

I will be getting married in Ukraine in October, and I cannot find an example of a "letter or certificate of Non-Impediment. I have looked on the US Embassy website in Ukraine, and the Ukraine Embassy in USA website. I also called the Ukrainian Embassy, and they said that I do not need to get the letter of impediment in Ukraine despite what the US website says. Also, I have a friend who got a letter in Raleigh NC at the office of vital records, and this is consistant with what the Ukrainan Embassy in DC told me to do (My local office of vital records) I just need an example of one so I can give it to the Office of Vital Records so they know what to type up. HEEEEELLLLLPPP!

  1. I am a US citizen and had to fill one out when I married my husband (a Tunisian). The letter is a form that gives basic information about you and is a sworn statement that you are not currently married. It states:

    * Your full name as it appears in your passport (last name, first name, middle name)

    *Your complete name at birth (last name, first name, middle name)

    *Date, city, state, and country of birth

    *Name of your father (last name, first name, middle name)

    *Name of your mother (maiden name, first name, middle name)

    *Your passport number, date of issuance, city and country where it was issued

    The next section refers to if you have bee married before. If you have not been married, then obviously you would leave it blank. If you have been married, then you need to fill out:

    *Date, city, state, country of marriage

    *Whom you were married to (last name, first name, middle name)

    *If the marriage was terminated by death, the date, city, state, and country of death

    *If the marriage was terminated by divorce, the date, city, state, and country of divorce

    You must bring your current US passport, a notarized copy of your birth certificate, and either a notarized copy of the death certificate or divorce decree if they apply.

    You are not allowed to sign it until you are before a consular officer at the embassy. They will verify all documents, sign it, and notarize it there. There is a fee to have this document notarized.

    Mine only took a matter of minutes to do. Just the time to fill out the form, swear in front of the consular that what I wrote is true and accurate to the best of my knowledge, sign it, and have the consular sign and notarize it.

    Mine is bilingual -French and English, but if you would like, I can scan a copy of mine and send it to you to show you what it looks like.

  2. You must get the Letter of Non-Impediment from the US Consular Annex in Kyiv. You fill it out right there (only on Friday mornings between the hours of 9 and 12) and swear before the consular official that what you have written is true. You then take this letter to the Department of Foreign Affairs in Kyiv to get it legalized (you pick it up after 4 that same day). As far as I know, you cannot get it in the US because it is specific for Ukraine. I just got one in August. It's a simple process, but it takes all day to get the letter from the consulate and then get it legalized at the department of Foreign Affairs. But you're already late if you want to marry in October because you will then have to take this document, along with your passport and a translation to the marriage office in the town where you plan to marry and they will then give you a license in 30 days.

    LATER EDIT:  I just looked at my certificate of non-impediment and you MUST get it in Ukraine because it is bilingual--English and Ukrainian.  You cannot get a copy done in the US.  If you take an English copy to Ukraine, you must still go to the US Consular Annex on Friday between 9-12 and fill out a new copy there.  You just have no choice in the matter.
