Where can I find legal forms online?

Hi,

This is for my daughter... when her son (4 1/2) was born, she had divorced his father, so she gave him her maiden name that she had taken back. They were married at the time of conception. When he was about 1, they did paternity testing that proves her ex-husband was the

father. He accepts being the father and gets their son every other weekend. They are on friendly terms and want his name changed. Where can I find legal name change forms on the internet?

  1. The forms and process vary by locale. Your local county/city courthouse will have the forms available for free. You just will have to ask a few questions to find out which office has them. You should be able to fill them out and file them for a small fee (probably less than a hundred bucks).
