If you know the manufacturer, and they're still in business, you can contact them.

Otherwise, your town likely has a glass store.  I live in a town of 100,000 and we have a glass store here that was able to cut a custom piece of glass for putting on top of my L-shaped desk.  You would just need to trace the shape on a large piece of paper, and tell them what thickness you want it, and if you want any special features like beveling, shatterproofness, etc.

Or if you just want a rectangle, you can try Home Depot or Lowe's, and they can cut it for you but you will likely only have one thickness to choose from there.
