Where can I find the correct 1306 form for an early out from the navy?

I don't want to hear the speech about ' oh you should stay in for the full enlistment ' . They are getting rid of my job and replacing us with civilians. I've been in for 3 and 1/2 years. The turnover to the civs happens in 2 months. In order to get out of the military, and not being stuck handing out basketballs for 4 months wasting my time, they're telling me to submit a 1306. The problem is the highest ranking person in my immediate command is an e4 and he doesn't really know what's up. I have a chief that is too busy for anybody and 400 miles away. I'll prob talk to my career coulencer in a week or two but if anybody here knows where to get the form to submit an early out ( 6 month early outs are common and you should tell ppl that who ask on here ) that would be really helpful.
