  1. You can order some Canadian birth certificates online, and those that cant be ordered online, the forms are available as well.  Here is a link to all the Canadian provinces birth certificate sites for forms/ordering:

    Most times you get your birth certificate faster if you order it online.  (if the choice is there for online order or sending in the form)


  2. Birth Certificates are issued by the province, so you have to deal with the province where you were born.  In most provinces it is handled by Vital Statistics, although the main one that isn't is Ontario where it is called Service Ontario.

  4. If you were born in Canada you can get an application online from the department of Vital Statistics from the Province in which you were born.

Birth certifcates are available through the provincial/territorial government of each province/territory.  De[pending on where you were born, you can contact the Vital Statistics (or equivalent) Agency in the province to apply for a birth certificate.  Many provinces now have online ordering for them so all you need is the information they request and a credit card to order one.  Since you do not say where you were born I am answering this as if you were born in Canada.  If you were born outside of Canada then the procedures may change.
