Where can I get an application form for EMA?

Is there a website that I can print it off from? I don't think there are any in my school and I don't really want to wait till I get into college to get an application form as this will probably mean I won't get the money right from the start.....

website address for EMA is www.direct.gov.uk/ema or you can call 08001218989 between 07.00 and 20.00 mon-fri. But your school should have some to give out. My daughter got hers last week from her head of year.
