Where can I get assistance for my sick mother (chicago)? Please help.?

Me and my sister are currently going through a guardianship battle. I am trying to get my mother home, whereas she wanted to be not in the nursing home ( she has alzheimers, and speech is a mess ). My Lawyer asked that I come up with a comprehensive plan as to who is going to take care of her 24 hours, their qualifications, how much they will charge, their hours of work. Is there anybody out there who know where to find such a person who can assist with my mother. I love my mother and don't want her in a facility, I want her cared for in her home, next to the people that loves her. I think 8-10 dollars an hour for night is fair.

Please help.
