Where can I get personalised children's clothing?

Our eldest (2 years) has a lovely cream knitted jumper with his name embroidered on the top of his chest over a blue fish. It is really cute, but the manufacturer has stopped making them.

Does anyone know of a place that does something similar or someone who would make these to order?
