  1. From your state election board or state state department.


  2. Go to your voter registration bureau and request it, it should be the same place where you registered to vote.  You should have been given a receipt when you registered, and received something in the mail with your precinct and voting locations on it within a few days of registering.

The county you live in handles voter registration and in my county it would be under the county clerk's office.  Other states call this position "county recorder."  We call the department that handles elections the "Bureau of Elections."  There is a second place you can get this information and that is the Secretary of State's office for your state.  I do not think there is any variation among the states in the election information you can get from the secretary of state.

  4. It depends upon your state but many states you can register at the DMV and they can also provide you proof. You can also go the local headquarters of whatever political party you belong to and they can pull you up and provide proof.

  5. There should be a Board of Elections in your community. They have proof. You may also have a voter registration card. Sometimes you are sent mail indicating the date of the next election and the location where you vote.

    You should be able to get duplicated of the first two items.

  6. Some states send out voter cards, but if you don't receive something in the mail you can always call your state's office and double check you are registered. You can search online for the contact information of your state's Secretary of State office.
