Where can I set up polls and have people respond to them?

Could somebody please tell me about a website where I could input polls and have the site feed data back to me? Preferably I could pose a series of questions to one respondant and have all of that person's data returned to me as one row of a data table. The website would have to be visited fairly frequently so I could gather a relatively large amount of data.
