The address of the doctor where you would have to go will be given to you by the U.S. Consulate General or Embassy, where you filed the application.

Just click this link from USCIS website:

Then enter your zip code, it will then give you a list of the appointed civil surgeon in your area.

You can find a doctor on the USCIS.gov website. They will search for doctors around your zip code. Make sure you call around though.. there's no set fee so the doctors make up their own price for the immigration exam. I paid $180 but someone I know paid $250.
