  1. Doing a lot of diorama and model-railroad scenery, I'd recommend 3M Styrofoam (extruded-polystyrene).

I find mine at the local Home Depot in 1 and 2 inch thick sheets.  I've also used foam from my local hobby-shop (smaller more expensive sheets)

The key to strength is to coat the foam after shaping and carving with PLASTER-CLOTH... Woodland Scenics sells a good package.  It's gauze impregnated with plaster, and when it dries it provides a nice strong shell and good base for painting and ground-cover.

    GOOD LUCK

  2. Call an Upholstery shop.

  3. Depends where you are.  In the US, Home Depot is apparently the place to go.  In the UK, either B&Q or Wickes will hold what you're looking for.

Ask at the trade desk for polystyrene insulation sheeting.  This usually comes in blocks of various sizes up to 4'x6' (perfect for a wargaming table).

There are a variety of versions and types including the famous "pink" and "blue" foams that are used by the GW Studio.  These are more expensive that the familiar white expanded polystyrene sheet, but much more robust.  In addition, it's a lot easier to cut and saw the pink and blue foam than it is white polystyrene (which tends to creat a h**l of a mess unless you use a hot wire cutter).
