  1. I was born in PA myself.  Go to the registry website for PA, and go to the county in which Montgomery is located.  They'll have an application to fill out, pay a small fee, and get the copy.

    Here you go....

  2. [look up health vital records]

  3. http://www.dsf.health.state.pa.us/health...

This will give you the instructions on requesting a new certificate.

  4. The state office of vital records.

  5. You look up on the Internet, the place for * vital statics* in PA

    http://www.vitalrec.com/pa.html#State

Vital Records, State Dept of Health

    P.O. Box 1528

    101 South Mercer Street

    New Castle, PA 16101

    (724) 656-3100

    Fax: (724) 652-8951

    Birth $10.00 (by mail)

    $17.00 (by fax)

You need to be able to say things, like your mother maiden name, fathers name,  name of town, or hospital,  some basic information,  but you should expect 3 to 6 weeks for it to be delivered by regular mail,  most places that want one,  want the seal on them, so you need to consider the mailed one, it will have the seal,

I hope that helps,  good luck
