Well SOAP notes are the correct way to chart.  I'll try and make one up for a massage therapist from my mind.

O. Soto Hall's test is positive

A.  Bilateral myositis of rhomboids

P. Deep tissue massage to the thoracic paraspinal muscles.  Triggerpoint therapy to the rhomboids bilaterally.
