  1. When I lived in the Bay Area

    (San Francisco/Oakland/Berkeley)

I used to laminate my pass to keep it intact.

You might see if you can get one from the place where the pass is issued; or any office supply store.

Another alternative is to go to a uniform store and see if they have ID holders, like the ones the cops use. Those are designed for heavy use and are ideal for transit passes.

  2. Office Depot, Staples, anywhere.

They probably sell them in packages of 50 or something though.

  3. I use to buy my MUNI fast passes from Hop Hing Ginseng at 1027 Stockton St San Francisco, CA and they always give you a free pass holder IF YOU ASK.  Not sure if they still do since I don't live in the area anymore.  If not, there's this 99 cents general goods discount store on Stockton St between Sacramento and Clay on the US Post Office side close to that 30 Stockton bus stop heading towards downtown that sells them for something like 25 or 50 cents each.  Its a clear plastic sleeve.

Otherwise, you can always ask the vendor who sold you the pass if they have any BEFORE buying the pass from them.  If they don't have any, simply go to another vendor.

My grandmother get hers from senior centers and health fairs.  I heard the 4th/King Caltrain station booth sometimes give em away too, though I have no confirmation of this.
