I think it came from the book "The Perfect Storm" which was later adapted into the movie.  The book, by Sebastian Junger, pre-dated the movie.  In the instance of that storm, it describes how 3 storms came together off the coast of New England to form "the perfect storm."  It really means an incredibly terrible storm at sea and is based on a true event.
