Depending on where you're from, either your nearest Ukranian embassy or consulate. Check their website for details. If you're a resident in a third country (you're a citizen of X, yet you live in Y), call them to see whether the embassy in Y or X is supposed to process your application.

Depends on where you are a citizen.

If a US citizen, or a citizen of the EU (and several other countries), you do not need a visa to visit Ukraine for up to 90 days at a time.

going out on a limb here;  but i would go online and look up kiev .com or something and you should be able to track down what you want there.
