Where do I get a "qualification questionnaire" for superior court jury duty?

I received a letter a while ago telling me I need to submit a questionnaire to the court for jury duty but it didn't include one, also I don't even live in the state where it was sent, so what should I do?

  1. Call the Clerk of court at the county specified.  And ask them.

    Perhaps the letter was sent in error?
