Where do I get my medical card? (UK answers only please)

I've got to provide various forms of proof of address for something and one of the things listed is "medical card". Where would I get hold of this, from my doctor? Thanks!

When u are registered with a GP, the health authority shld have sent u one to yr home address.

They send you it via post if you haven't received it yet, give it another week then phone your doctors surgery and ask about it.
