Where do I report malicious websites AND software?

Well I use McAfee Site Advisor and you can report it to them.

I have it installed on Firefox and at the bottom of the page, it turns green if it is safe and red if it is not safe. It also shows up in Google search results before you click on the page.

Your antivirus should have a file a report button.

The great thing about the Internet is that it really doesn't have a lot of regulation.  Unless the site is doing something illegal, just avoid that site.  If the site is part of a hosted service, contact that ISP and have them review the content to see if they'll discontinue working with that customer and the site will go away (at least until they find another host for it).
