I need to know where to file complaint before filing charges against an insurer

It depends on why benefits were denied. Was is pre-service, like an authorization? Or was it a claim denial after the service was provided? Was it denied because its not covered? or not medically necessary? Be more specific if you need a better answer than this...   If the insurance is through an employer group, file the complaint with the insurance company. If its private insurance, file it with the department of insurance in the state the policy was written in.
