  1. doctor

  2. Ask your parents where the documentation is, as they would have received documentation stating you have dyslexia, as well as some tips on how best to help you when in class.

If they don't have it or have lost/misplaced it you will need to see a specialist again, for a full assessment.

Typically a educational (school) psychologist or a specialist teacher, who is trained in diagnosing Learning disabilities and dyslexia, and they will also implement strategies such Multisensory Instructional Methods to assist you in the class room.

A regular doctor can not or is not trained in diagnosing Dyslexia, however they will send you on to a educational (school) psychologist, firstly they will ensure you have had yours eyes and vision checked for any problems, once ruled out, they will then do some testing of your written work, spelling, maths etc, they will also visit your school, and watch you in the class room, talk to your teachers/parents, and look at previous work you may have handed in.

Good luck and hope you can get some help

  3. How do you know that you're dyslexic?  If you were diagnosed as that, then get the documentation from your therapist or doctor.

  4. You need to have undergone a dyslexia screening, test and then be diagnosed by a dyslexia testing specialist. The specialist that you've seen should have the proper documentation.

  5. From the medical person who diagnosed you.

  6. If you're in hs, you should go to your school counselor for testing.  If you're in college, you need to go to disability services.  You'll be able to take a test for LDs and learn what accomodations your school is required to make for you if you indeed have a one.  It's certainly not sufficent for you to go around telling people that you have dyslexia.  You will need some proof.  The good news is, if you do have a documented disability, you'll probably be elligible for tutoring, extra time for assmts, etc so you can bring up your gpa.  Good luck

  7. medical record , if you don't have it you can buy it from some doctors.

  8. your family medical record
